We accept all Colorado Medicaid Regional Accountable Entities, including:

Important: You must inform us if you have Medicaid before beginning services. Medicaid members generally cannot privately pay a Medicaid-enrolled provider for services that may be covered by Medicaid.
Commercial Insurance
Congruency Counseling currently accepts select commercial insurance plans, including:

UnitedHealthcare
Insurance participation may vary by therapist. Acceptance of an insurance company does not guarantee that every service or therapist is covered under your specific plan.
Private Pay
Private pay is available for clients who do not have insurance, choose not to use insurance, or are receiving services that are not covered by their plan. Rates vary depending on the therapist.

Before Your First Appointment

Before scheduling, contact your insurance company and ask:
Is my therapist in-network?
Do I have outpatient mental-health benefits?
Do I have a deductible?
What is my copay or coinsurance?
Do I need prior authorization?
Are telehealth sessions covered?
Is couples or family counseling covered?
How many sessions are allowed?
Insurance verification is not a guarantee of payment. Clients are responsible for understanding their benefits and for any balance not paid by insurance.
Out-of-Network Benefits
Using a superbill
When Congruency Counseling is not in network with your insurance plan, we may be able to provide a superbill for you to submit to your insurer for possible reimbursement.
You are responsible for paying the full session fee at the time of service. Reimbursement is determined by your insurance company and cannot be guaranteed.
Before scheduling, consider asking your insurer:
Does my plan include out-of-network mental-health benefits?
What percentage of the session fee may be reimbursed?
Do I have an out-of-network deductible?
How do I submit a superbill?
Are there restrictions on diagnosis, provider credentials, or telehealth?
Good Faith Estimates
Your right to a Good Faith Estimate
Clients who do not have insurance or who choose not to use insurance have the right to receive a written estimate of the expected cost of non-emergency healthcare services.
You may request a Good Faith Estimate before scheduling or at any point during treatment. When an appointment is scheduled sufficiently in advance, the estimate should generally be provided within the time required by federal law.
Make sure your health care organization gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care organization, and any other organization you choose, for a Good Faith Estimate before you schedule an item or service.
If your final bill is at least $400 more than the amount listed on your Good Faith Estimate, you may have the right to dispute the charge.
Make sure to have a copy or a picture of your Good Faith Estimate.

Contact your insurance company using the phone number on the back of your insurance card. Ask whether Congruency Counseling and/or Melissa Frick are in network with your plan. Because our interns and LPCCs bill under Melissa Frick's license, they may not appear individually as in-network providers. If you ask about a therapist by name, your insurance company may incorrectly state that they are out of network. Verifying coverage under Congruency Counseling or Melissa Frick will provide the most accurate information.

You may be responsible for the unpaid balance. Our office can help explain billing information, but the insurance company makes the final coverage decision.

You must disclose Medicaid coverage before services begin. Medicaid has specific restrictions regarding private payment for potentially covered services. We will help determine what options are legally and ethically available.

Clinical supervision and professional consultation are generally separate from psychotherapy and are typically not billed to medical insurance.

Please bring your insurance card, a photo ID, and any required intake forms. It can also be helpful to have a list of current medications, relevant medical or mental-health history, and any questions you would like to discuss with your therapist.

Session rates will be discussed with you during your initial conversation, before your first appointment is scheduled, so you know what to expect before beginning counseling.
If you plan to use insurance, your out-of-pocket cost may also depend on your individual plan, deductible, copay, or insurance.
